چکیده

Atrial tachy-pacing was performed in 34 patients who were divided into 3 groups as follows. Group I: 4 patients with obscure chest pain syndromes and normal coronary arteries and haemodynamics; Group 2: 25 patients with significant coronary artery disease and angina pectoris; Group 3: 5 patients with coronary artery disease, left ventricular aneurysm, and left ventricular dysfunction. Pacing produced a reduction in the left ventricular stroke work index and the left ventricular end-diastolic pressure in Group i patients, while the cardiac index and the left ventricular minute work index showed no significant change. Group 2 patients showed a variety of responses. Nine patients had a 'normal' response to atrial pacing, namely a fall in left ventricular stroke work index and left ventricular end-diastolic pressure. Of these, .5 had experienced angina during pacing and 4 did not; the other I6 had abnormal responses. When considered as a group, there was a significant decrease in the left ventricular stroke work index and an increase in the left ventricular minute work index, while the left ventricular end-diastolic pressure and cardiac index did not change significantly. Group 3 patients demonstrated a significant fall in the left ventricular end-diastolic pressure and a rise in the cardiac index and the left ventricular minute work index, while the left ventricular stroke work index was unchanged. A reduction in the end-diastolic pressure without a fall in stroke work index and with an increase in the minute work index indicate improved myocardial performance in this group ofpatients. It is concluded that there is a wide spectrum of responses to atrial pacing ranging from no change in left ventricular function in some patients to gross dysfunction in others. Those with compromised left ventricular function exhibit improved performance.

Haemodynamic events in right and left ventricle during angina induced by atrial pacing.

Atrial pacing to the point of angina or up to a maximum rate of 166/min was carried out in 37 patients during coronary angiography. In 9 patients with normal coronary arteries both the right and left ventricular end-diastolic pressures fell remained steady with increasing heart rate. The same response was observed in 14 patients with angiographically proven coronary artery disease. In the rema...

Beneficial effects of diltiazem in coronary artery disease.

The effects of the new calcium channel blocking agent diltiazem were evaluated in 11 patients with stable angina pectoris and confirmed obstructive coronary artery disease at rest and during rapid atrial pacing. Symptomatic, metabolic, coronary, and systemic haemodynamic indices were monitored at rest and during pacing induced ischaemia. At rest, after the administration of intravenous diltiaze...

Comparison of atrial pacing and the cold pressor test in patients with angina pectoris.

A haemodynamic and myocardial metabolic study was performed to compare effects of maximal atrial pacing and the cold pressor test (CPT) in patients with angina pectoris. Twelve patients (group I) had angiographically severe coronary artery disease, while 16 patients (group II) had normal coronary angiograms. At maximal pacing, angina developed in all patients in group I, and myocardial lactate ...

Incidence of Atrial Fibrillation after Coronary Artery Bypass Graft Surgery and Its Risk Factors in Shiraz, Iran during 2017-18

Background and Objectives: Atrial fibrillation is one of the most common disorientations after coronary artery bypass graft surgery and can affect the chance of survival in the first year after surgery. The present study aimed to determine the incidence of atrial fibrillation after coronary artery bypass graft surgery and its risk factors.   Methods: This cross-sectional study was performed on...

Haemodynamic comparisons of atrial pacing and exercise in patients with angina pectoris.

Though exercise is the most physiological test for angina pectoris, it has several disadvantages. The most important are that, it is difficult to achieve a steady state, comparable haemodynamic parameters are not readily reproducible during repeated periods of exercise (Burkart, Barold, and Sowton, 1967), it cannot be used effectively in patients with poor exercise tolerance, and angina pectori...
